Your procedure will take place at one of our two facilities, the Florida Campus or the Griffin Campus. If you are unsure of your location, please check our physician listing to confirm the correct facility. If you are still unsure, contact your physician to see where you are scheduled.
What is the process to check in at the surgery center?
All patients are required to have a negative Covid- 19 test which is coordinated through the physician’s office. Upon checking in, patients will be screened and face masks are required. You will see all staff wearing a face mask as they are ensuring compliance with our Covid-19 re-opening policy. It is our commitment to safety to mask anyone during this time. Rest assured that your healthcare providers are in good health, we screen and check temperatures daily and we actively encourage staff to stay home if they have symptoms or are ill. Patients who require special assistance and minors may have only one (1) visitor to obtain vital clinical history/information.

Translation services are available for non-English speaking patients and their families.
Prior to Your Procedure – Complete Your Patient Health Form
As soon as your procedure has been scheduled, complete your Patient Health Form. Once you do this, our nurses will be able to access the information you entered. This information will assist the clinical team in preparing for your procedure.

NOTICEIf you have an Advance Directive that contains a “Do Not Resuscitate” provision, it will be suspended while at Lakeland Surgical & Diagnostic Center.
Notes to Visitors
LSDC is a smoke-free facility. Please refrain from smoking on-site. Please also refrain from smoking and using tobacco products before and after the patient’s procedure.
We ask that you be respectful with cell phone and tablet use in the waiting room. As a consideration to other patients and guests, we ask that you use earbuds and step outside to make phone calls.
Each patient and visitor entering a Lakeland Surgical & Diagnostic Center campus must comply with the following:
• Wear a face mask or cloth covering that covers the entire mouth and nose and must always be on while in the facility. A nurse will provide a mask as needed.
• Complete screening each time entering the facility, and disclose, any symptoms of illness including fever, shortness of breath cough, sore throat, body aches, chills, new loss of smell or taste, nausea, vomiting or diarrhea.
• Practice social distancing by remaining 6 feet apart from others whenever possible.
• Visitors are not allowed in the facility. Visitors may either wait outside in automobile and/or give their cell phone numbers to admission staff and nursing staff will call driver upon discharge.
